Open Enrollment will be from October 1 @ 7:00 AM to October 30 @ 4:00 PM. All changes will be effective January 1, 2025. Benefit hours are 7:00 AM to 4:00 PM, Monday through Friday. Forms can also be emailed with all supporting documentation to : OpenEnrollment@umcsn.com . Please contact our office if you have any questions.

The two health plan options are Clark County Self-Funded PPO and Clark County EPO.

If you would like your healthcare benefits or supplemental life to remain the same, no action is required.

Flexible spending account (FSA/TASC) requires annual reenrollment. If you wish to continue to participate in FSA please complete the 2025 FSA enrollment form.

You may review your current benefits under Employee Self Service (ESS) – Benefits (Display Benefit Plans) – Download button on bottom of page – this will take you to a benefits confirmation page.

Below are the different types of changes that can be made during Open Enrollment and the backup documents required.

Switching Health Plans ( Going from Self-Funded PPO to EPO or EPO to Self-Funded PPO)

Please complete the following:

OE Switching Health Plans Form
Backup documentation required: Copy of social security card for yourself and your dependents (If applicable), and copies of your certified marriage certificate (If applicable), and birth certificates for dependent children (If applicable).
For Both Plans: Clark County COB Form PPO - EPO

Waived to Benefited

Please complete the following:

Open Enrollment Benefit Packet
Backup documentation required: Copy of social security card for yourself and your dependents (If applicable), and copies of your certified marriage certificate (If applicable), and birth certificates for dependent children (If applicable).

Benefited to Waived

Please complete the following:

OE Waived Benefits Form

Adding to Self Funded Plan PPO / EPO, dependent or spouse

Please complete the following:

Sun Life

During Open Enrollment, you can increase your supplemental life insurance, by 3 increments (up to $30,000) up to the Guaranteed Issue amount without going through underwriting. If you have do not currently have supplemental insurance or are requesting anything over the guaranteed issue amount, you will need to apply through our office. We will then direct you on how to complete the Evidence of Insurability (EOI) process.

Guaranteed Issue is as follows:
Employee - $250,000
Spouse - $30,000
Children - $20,000 ( during open enrollment, child supplemental life can be added without going through the EOI process)

Accidental Death & Dismemberment (AD&D) – Can be added up to $300,000 during Open Enrollment without going through the EOI process

If adding supplemental life for the first time for dependents:
Copy of social security card for your dependents (If applicable), and copies of your certified marriage certificate (If applicable), and birth certificates for dependent children (If applicable).
